Reports to: Director of Clinical Services / Director of Admissions
Overview of the position:
The PASRR Clinical Evaluator is responsible for conducting comprehensive clinical evaluations (Level I and, when indicated, Level II) of potential residents to determine mental health, intellectual disability, or related conditions in accordance with Tennessee's PASRR (Pre-Admission Screening and Resident Review) requirements. The evaluator assesses suitability for placement in a nursing facility and determines need for specialized services to ensure both regulatory compliance and person-centered care planning.
Duties and Responsibilities:
Administrative Functions:
Conduct Level I screens for all applicants in compliance with Tennessee regulations.
Perform Level II in-depth evaluations for individuals who screen positive, including in-person assessments, interviews, collateral information collection, and functional assessments.
Assess psychological, behavioral, and cognitive functioning to identify SMI, intellectual/developmental disabilities, or related conditions.
Prepare clinical summaries and level-of-care determinations.
Recommend appropriate placement and any specialized services needed.
Collaborate with facility leadership, nursing, social work, and clinical staff to integrate evaluation results into admission planning.
Ensure all documentation meets federal (42 CFR Part 483) and Tennessee regulatory requirements.
Submit Level II outcomes and recommendations to the state-designated PASRR contractor.
Maintain accurate records of assessments, determinations, and service recommendations.
Monitor evaluation turnaround times to meet state/contractor benchmarks.
Reassess individuals when significant clinical changes occur or as required by policy.
Provide education or training to facility staff on PASRR processes and care considerations.
Serve as a liaison between the SNF, PASRR contractor, TennCare, and other stakeholders.
Communicate evaluation results and recommendations to residents, families, and the interdisciplinary team.
Participate in meetings related to admissions, care planning, and regulatory reviews.
Education, Certification and Experience Requirements:
Bachelor's degree in Nursing, Social Work, Psychology, Counseling, or related behavioral health field required. Master's degree preferred.
Must hold a valid Tennessee clinical license (RN, LCSW, LMHP, or equivalent).
Minimum 2 years in clinical assessment of adults with mental illness, intellectual/developmental disabilities, or related conditions; experience in long-term care or behavioral health preferred.
Knowledge of PASRR regulations (federal and Tennessee-specific) and long-term care level-of-care criteria.
Strong assessment, critical thinking, documentation, report-writing, and communication skills; ability to work independently.
Ability/willingness to travel for in-person assessments; computer proficiency for documentation and data submission.
